Types of Visas and Cost

Required Documents:
- Valid Passport: a passport valid for travel to the United States and valid for at least six months beyond the date the applicant will exit the United States.
- 1 photograph: a photograph, 5cm X 5cm on a white background, taken within the last six months.
- Application Form DS-157: Only for male applicants between the ages of 16 and 45
- Application Form DS-158: Only for students and exchange visitors
- Visa Application Processing Fee $131 is payable in U.S. dollars or the equivalent in new Azerbaijani manats. This fee is non-refundable.

- Complete an electronic visa application form No DS-156 (for tourists-short stay)
- Make an online appointment
- Attach necessary documents
- Come to embassy to apply for a visa
Days of acceptance for visa applications are 4 days a week: Monday, Tuesday, Wednesday, and Thursday, between 9:00 a.m. and 12:00 noon, only by prior online appointment at allocated time. The Consular Section does not accept appointment requests via the telephone. Applicants must make appointments themselves via our online appointment system. Please note that applicants are first required to fill out the application form online and will have to use the barcode number on the 3rd page of the application to schedule an appointment. You must print out your online appointment confirmation page to gain an access to the Consular Section.
